VNA Heath System has partnered with two globally recognized leaders in ECG technology to develop the cutting edge, Hearts at Home CHF Program. Hearts at Home is designed to improve patient care, reduce readmission rates, and eliminate the need to transport patients to a physician office or hospital.
Mortara Instruments, mortara.com, a world leader in ECG Technology, provides the hardware--an elegantly designed, hand-held, lightweight, 12 lead in-home electrocardiograph. The unit is equipped with a wireless SIM data card that transmits instantly to our server.
Epiphany Cardiography Products is a global provider of software, which simplifies the collection and management of ECG data. Epiphany's software eliminates the need for dedicated client software. It can compare suspected STEMIs with previous ECGs anywhere, even on a golf course with an iPhone.
VNA Health System provides and supports a HIPPA compliant Cardio WiFi Hosting Service, which allows for remote access to cardiac data by physicians and agency personnel. The server will also house program guidelines and collaterals for agency use.
VNA Health System has incurred significant start up costs in an effort to improve the level of care provided to our patients. Unfortunately, no reimbursement opportunity exists to recover these costs. Our hope from the onset has been to engender sufficient interest in the program from within our VNA community to share in the cost structure, making it affordable for all, including ourselves.
We are introducing Hearts at Home, preliminarily to our fellow VNA's at the Mortara booth at the VNAA Annual Meeting. The more official presentation, including white papers, will occur shortly following the annual meeting via a webinar presentation made available to VNAA members.
